New patient registration

Use this service to register with the surgery.

We strongly recommend that all members of the same family or household register with the same GP surgery.

You can use this service if you:

• want to register at the surgery
• live within our catchment area

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• basic health and medical information
• other personal details and demographics
